*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per loaded mile fee. Toll charges not included.
*Additional fee of $100 per additional person required for transfer.

2. Transfer to Airport… $300
*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per mile fee. Toll charges not included.
*Wait time is an additional $45 per hour.

3. Transfer to Crematory… $250
*This fee is per deceased transfer to crematory.
*Toll charges not included.

Professional Care Services
1.Professional Embalming… $300

2.Professional Embalming of Autopsied Remains… $500

3. Restorative Care… $100 per hour

4. Bone/Tissue Donation… $100

5. Topical Disinfection… $150

7. Dressing/Casketing… $150

8. Hairdressing…$100

Arrangements
1. At Need Arrangements… $250
Plus 3% of merchandise sold.

Maintenance
1. Care Center/Prep Room Sanitation… $250
Complete cleaning and sanitation of care center. Includes: Disinfecting embalming table, dressing tables and utensils. Cleaning and organizing cabinets. Steam and clean care center floor, counter tops and waste bins. Dispose of garbage. (This does not include disposing of medical waste).

2. Care Center Management… $400 per month
Includes: care center/prep room sanitation, inventory care, embalming equipment maintenance.

Funeral Services/Livery
1. Hearse… Funeral Homes Rate
*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per mile fee.

2. Limo… Funeral Homes Rate
*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per mile fee.
*Wait time is an additional $45 per hour.

3. Pallbearer… Funeral Homes Rate

4. Funeral sign-in… $200
*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per mile fee.

5. Directing Funeral… $300
*Local area- 35 mile radius; distances over 35 mile radius there will be a $3.50 per mile fee.
*Wait time is an additional $45 per hour.

6. ASD Phone Coverage… $200 per 8 hours.

*If a transfer comes in while on call for you, the $200 ASD phone coverage fee will go towards your first transfer of the night

7. Visitation/Memorial Service Setup & Walk-in… $200
